When is my premium payment due?

Health insurance premiums are due on or before the first of the month when coverage begins or renews and the first day of each calendar month after that.

Where do I send my premium payment?

Look on your premium invoice for the address and instructions on how you can pay, including the mailing address for premium payments.

What is my outstanding premium due?

My invoice does not reflect my most current payment.

Your premium payment may have been received and processed after the most current invoice was produced. Your next invoice will reflect your payment. If you have questions, large group employers can contact billing services at 866-204-3132 or, if your small group plan is administered by one of the following, contact them with billing questions:

My invoice is billing for subscriber(s) who have been terminated.

In some cases, enrollment transactions are processed after invoices are produced. Please pay your invoice as billed and your next invoice should reflect any subscriber terminations.

My invoice does not reflect all active subscribers.

Sometimes enrollment transactions are processed after invoices are produced. Please pay your invoice as billed. Your next invoice should reflect any subscriber additions.

What transactions can I perform online?

You can submit address and name changes, add and terminate subscribers and dependents, view premium payment history, and more.

How long will it take for my changes to be updated in administrative systems?

Transactions are typically processed within 24 hours at ConnectiCare. Small group employers, check with the distribution partner who administers your plan.

Where do I find forms?

Why did my enrollees receive 1099-HC forms?

Health plans are required to mail form 1099-HC to Massachusetts residents who are enrolled each year.

What is the FID number of insurance co. or administrator on ConnectiCare’s 1099-HC (box 2)?

I have an enrollee who is a Massachusetts resident and was enrolled last year but did not receive a 1099-HC.

Enrollees must have had credible coverage with us at some time last year with a Massachusetts home address to automatically receive a 1099-HC. We can generate and mail a 1099-HC forms on demand.

I have an enrollee who works in Massachusetts but does not live in Massachusetts.  Does this person need a 1099-HC?

This law covers Massachusetts residents only. ConnectiCare mails 1099-HC forms to Massachusetts residents only. Please contact the Massachusetts Department of Revenue at www.mass.gov/dor/hcinfo or 800-392-6089 for more information. An enrollee may contact ConnectiCare at 800-251-7722 to ask for a 1099-HC form.

A covered dependent who does not live with a plan subscriber needs a 1099-HC for their own tax filing. How can the dependent get a separate 1099-HC?  

1099-HC forms are generated for subscribers only. Subscribers can share the 1099-HC form with their dependents or ask us to mail the dependent a copy.  An enrollee may contact ConnectiCare at 800-251-7722.

My coverage did not end on December 31. Why does the form have an end date of December 31st (last year) in the "Coverage Thru" field?

This form reflects coverage for the tax year on the form. It does not reflect current insurance coverage.

A dependent is covered under an ex-spouse's plan. How can the dependent get a 1099-HC?

The individual can contact ConnectiCare at 800-251-7722 to ask for a 1099-HC form.
